Pre-Consultation
- Develop a draft Personalised Family Plan using information provided in the completed Family Information Form.
Consultation
- Conduct a home visit for the consultation.
- Discuss current behavioural challenges and strategies previously used.
- Explore parenting styles to empower parents/carers in shaping their Personalised Family Plan.
- Collaboratively refine the Family Plan with input from parents/carers.
- Introduce tailored strategies and resources to support the plan’s implementation.
- Offer guidance on accessing additional support services, if needed.
Post consultation
- Finalise the Personalised Family Plan.
- Deliver the completed plan via email or post.
- Provide follow-up support via email or telephone for 10 days after the consultation to monitor progress and address any concerns.
Fee: for all the above £125
Optional Add-On: Additional Support Package
- Includes a 30-minute follow-up telephone consultation to review progress and address any ongoing concerns.
- Continued email/telephone support for an additional 7 days.
